2021-2022 Florida Department of Education (FDOE) Annual Exceptional Student Education (ESE) Parent Survey

The survey is open to all parents of children in Pre-Kindergarten through 12th grade with an Individual Educational Plan (IEP) who are receiving ESE services. The survey is not intended for parents of students who are receiving only gifted services through an Educational Plan (EP). The FDOE Annual ESE Parent Survey response window is open now through Tuesday, May 31, 2022.   Complete the survey online at  The survey is easily accessible via computer or smart phone. Once the survey is completed online, submission of the paper survey is no longer necessary.

Mandatory Immunization Requirements for the 2022-2023 School Year

Florida Statute 1003.22 and Rule 64D-3.046 of the Florida Administrative Code, and school board policy 5320 mandates that children entering, attending, or transferring into school receive the appropriate grade level vaccines.   The Florida Department of Health in Miami-Dade County would like to inform parents/guardians who have students in PRE-KINDERGARTEN, SIXTH grade, and students with an EXPIRED Immunization status that their child/children are due for grade appropriate vaccines. Students entering Kindergarten Fall 2022 must obtain the 2nd MMR (Mumps Measles and Rubella) and the 2nd Varicella vaccine. Students entering the 7th grade Fall 2022 must obtain the Tdap booster (tetanus diphtheria pertussis) vaccine. Students in ALL grades with an expired immunization status must present a new Florida Certificate of Immunization-680 form. Students will not be permitted into school without the appropriate vaccine requirements
