The Florida Department of Health in Miami-Dade County (DOH) would like to inform parents/guardians who have students in PRE-KINDERGARTEN, SIXTH grade, and students with an EXPIRED Immunization status that their child/children are due for grade appropriate vaccines.
- Students entering Kindergarten Fall 2023 must obtain the 2nd MMR (Mumps, Measles, and Rubella) and the 2nd Varicella vaccine.
- Students entering the 7th grade Fall 2023 must obtain the Tdap booster (Tetanus Diphtheria Pertussis) vaccine.
- Students in ALL grades with an expired immunization status must present a new Florida Certificate of Immunization-680 form.
