FDOE Annual ESE Parent Survey

Each year, the Florida Department of Education (FDOE) conducts a survey to collect information for the FDOE and the United States Department of Education, Office of Special Education Programs (OSEP), regarding the quality of services and support provided by all school districts to parents of students with disabilities (SWD). The survey is open to all parents of children in Pre-Kindergarten through 12th grade with an Individual Educational Plan (IEP) who are receiving ESE services. The survey is not intended for parents of students who are receiving only gifted services through an Educational Plan (EP). The response window for completion of the FDOE Annual ESE Parent Survey is open now through Friday, June 30, 2023.  FDOE Annual ESE Parent Survey responses will be collected online through the links listed below.
